Booking of First Appointment for Nurse Interview

Referral by a registered doctor or psychologist is required for receiving assessment services in CACs. Parents should approach CAC by phone or in person for Booking of First Appointment for Nurse Interview.

Registration Checklist

Please bring along the following documents and fees

  • Referral letter from a registered medical practitioner or clinical / educational psychologist. Valid period of referral for new case booking is within 6 months from the issued date (must be orginal)
  • Birth certificate of the child (must be original copy)
  • Identity cards of both parents (photocopies are accepted)
  • One recent passport photo, MCHC Child Health Record and all specialist follow-up cards
  • Local residential address proof (photocopies are accepted)
  • Specialist outpatient service fee (HK$135 for first appointment and HK$80 for each subsequent consultation) or identity proof for civil service eligible person or valid Certificate of CSSA Recipients (for Medical Waiver)

*During every appointment afterwards, parents should bring along the birth certificate/identity card of the child for checking.

*Parent should inform Child Assessment Centre (CAC) as soon as possible for re-scheduling if the child are not able to attend the original scheduled appointment. Otherwise, the appointment would be cancelled. Parent would need to call CAC to make an appointment again.
